Overtime Girls League Player Waiver

I, the undersigned, acknowledge that participation in the Overtime youth girls' basketball league involves inherent risks, including but not limited to physical injury, accidents, and other unforeseen events. I understand that these risks may arise from my child’s actions, the actions of others, or the conditions of the facilities and equipment used.*

In consideration of my child's participation in Overtime Girls league, I hereby release, waive, and discharge the league, its officers, coaches, volunteers, and any affiliated organizations from any and all claims, liabilities, or damages arising from my child's participation in league activities, including but not limited to practices, games, and travel to and from events.*

In the event of an emergency, I authorize league officials to secure medical treatment for my child if necessary. I understand that I will be notified as soon as possible in case of such an emergency.*

I have read and understood this waiver and release of liability. I agree to its terms and acknowledge that I am signing it voluntarily.*
